FWIW, you do not need to disable UAC, although it shouldn't matter if you do.

I saw the same problem when I tried it just now, but when I set PokerStars to Run As Administrator as well as PT3 it is working fine. Sometimes software updates can make it forget settings like Run As Administrator so please check, although if you are always running with right-click > Run As Administrator that shouldn't apply.
I have Windows 7, not Vista, but I wouldn't expect there to be a significant difference for this kind of thing.
When you start PT3 and PokerStars with Run As Administrator, with UAC enabled, it should ask you to confirm twice (once each) - are you seeing that?

I now actived UAC again, rebooted and restarted both programs (both run as admin).
hotkeys now worked with pt3 open and not importing
hotkeys now also worked when importing. perfectly!!!! w00000000000000000000t

I just don't understand what the issue was, but I am assuming that is was related to the stars update. it had some settings messed up for me yesterday (table layouts that I had set as defaults, bet amounts, avatars were enabled etc). when I started stars today, it was back to my defaults and all changes I made to the hotkey assignments yesterday were not there anymore.

You're welcome - I'm glad to hear it's working again. Sometimes software updates can make it forget settings like that so it's always worth double-checking if something doesn't look right.
